Overview

 Key responsibilities of this role include:

  • Providing advanced clinical assessment, diagnosis, treatment and care planning for patients within scope of practice.
  • Ordering and interpreting appropriate diagnostic and laboratory tests and explaining procedures, treatment and care options to patients and whanau.
  • Managing your own caseload effectively, while working as part of an inter-disciplinary team to improve outcomes for patients and whanau.
  • Providing clinical leadership, education, consultation and mentorship to Nursing and wider healthcare colleagues.
  • Working alongside the Whanau Ora Navigator Team to provide clinical advice and support for patient cases.
  • Supporting external contracted services, which may include Youth Services, School-Based Health Services, Drug and Alcohol Early Intervention, Hearing and Vision, Mama, Pepi, Tamariki, Walk-in Clinic Support and Smoking Cessation services.
  • Contributing to practice improvement, quality case management, wellness planning and positive health outcomes for individuals and whanau.
  • Demonstrating cultural awareness and sensitivity, and supporting care that reflects the whanau ora model.

 Applicants will have a strong background in advanced Clinical Nursing practice, with the ability to work independently while also contributing positively to a multi-disciplinary team.


Requirements

Minimum requirements of the role:

  • Registered as a Nurse Practitioner with the Nursing Council of New Zealand and hold a current Annual Practising Certificate.
  • Ability to prescribe medication
  • A minimum of seven years’ recent clinical experience.
  • Advanced clinical assessment and patient management skills.
  • Strong knowledge of current Nursing issues, medico-legal and ethical responsibilities.
  • Excellent communication, interpersonal, problem-solving and planning skills.
  • Demonstrated clinical leadership and the ability to support and mentor others
  • Experience in community and/or whanau ora care would be an advantage.
  • Ability to work with external agencies and link patients and whanau with appropriate community resources.
