This is a core Community & Cultural Support role within the Ngāruahine Primary Care team supporting a relationship-based approach to meaningful engagement utilising our Whānau Hapai model of care in achievement of greater understanding and management of Diabetes and will work collaboratively with our Māori Health partners of Ngāti Ruanui Healthcare, Tui Ora and the wider Diabetes Integrated Team within Te Whatu Ora that aims to support whānau to improve their self-management of their diabetic condition and journey to enhanced wellbeing.
This role will work alongside our existing Kaitautoko which has specific focus predominantly supporting enrolled patients of the Ngaruahine, Avon Medical Centre, Eltham Health Centre, Opunake ‘Coastal Care’ and ‘The Practice’ based out of Hawera Hospital.

- focus to improve access to your rights
- strong linkages to hospital and specialist services, including strategies to support hospital avoidance and enhanced community care.
Your ability to demonstrate highly adaptable engagement styles prioritising Whānau Maori and Vulnerable or At-Risk Communities is key to your success.
