We are currently seeking a Mate Huka (Diabetes) Kaitautoko / Health Coach to join our team for an initial fixed term period of six months (30 hours per week), with the potential to move to a permanent full-time role, reporting to the Kaitautoko Lead.  This role will be based within our community premise on Victoria Street in Hawera. 


The ideal applicant will have strong connections to Taranaki and a genuine passion for improving health outcomes for Maori by offering support that is culturally appropriate. The successful candidate will have a solid understanding of tikanga and Te Reo, along with the skills and attitude to engage effectively with Maori. They will be enthusiastic about working in a collaborative team focused on finding new and better ways to achieve results.


Overview

This role supports whanau living with or at risk of Mate Huka (Diabetes) by providing culturally responsive coaching, education, advocacy and navigation support. The successful applicant will work alongside whanau to build confidence and capability in managing their wellbeing and achieving their health goals.  The Mate Huka Kaitautoko / Health Coach will:

  • Support whanau to better understand and manage Mate Huka (Diabetes) and related health conditions.
  • Encourage healthy lifestyle choices including nutrition, physical activity, smoking cessation and overall wellbeing.
  • Assist whanau to access appropriate health services, screenings, education programmes and community resources.
  • Work collaboratively with clinical teams, GPs, Nurses, Kaiawhina and external providers to improve health outcomes.
  • Provide ongoing motivation, support and practical guidance to help whanau achieve self-management goals.
  • Deliver support in a way that reflects kaupapa Maori values and whanau-centred practice.


Requirements

To be successful in this role you will:

  • Take a collaborative approach that seeks to connect with the community and whanau networks.
  • Have a foundational base of cultural competency and willingness to continually grow it.
  • Have outstanding communication and people skills.
  • Possess exceptional written and verbal communication skills.
  • Have knowledge or experience relating to Diabetes, chronic condition management, hauora or community health support (preferred but not essential).
  • Hold a full current Driver Licence.
